DO I NEED TO ENROLL IN A MEMBERSHIP PLAN TO BE SEEN AS A PATIENT?


YES. All patients who want Dr. Edwards as their primary care doctor are required to enroll in one of our membership plans. An affordable, low monthly fee applies.




CAN DPC (DIRECT PRIMARY CARE) REPLACE HEALTH INSURANCE?


No. It does not. You are recommended to maintain insurance for hospital visits, etc.




WHAT IS CONCIERGE MEDICINE?


Concierge medicine provides patients with enhanced and efficient experience that treatment seems beyond the doctor's office. Members pay a bill monthly fee, around the cost of a cell phone bill to have services provided that are not covered by insurance. In addition to this fee, your insurance is billed and you are responsible for any co-pays, call insurances and deductibles. You may also use your insurance for lab work, imaging and hospital stays.




WHAT IS DIRECT PRIMARY CARE?


Direct primary care is a new type of outpatient medical care that has been on the rise for the last 5 to 10 years. It completely skips billing the insurance company and charges patients one flat fee per month that covers a large amount of outpatient services such as office visits, some preventive testing and bloodwork. The doctor is able to work with vendors to find you the most affordable pricing for imaging and blood work.




CAN I CANCEL MY MEMBERSHIP?


Yes. Providing 30 days notice, you may cancel either your DPC or concierge membership in writing with an original signature. The concierge program requires a 3 month enrollment prior to cancellation. The DPC membership requires a 1 year committment prior to cancellation.




CAN I REJOIN AT A LATER TIME?


Yes but a re-enrollment fee and a waiting period of 6 months is required.




CAN THE PRACTICE DISCONTINUE MY MEMBERSHIP?


Yes, in the case of failure to pay for concierge services, fraud, failure to comply with treatment plans, two or more no-shows or abusive behavior toward Dr. Edwards or her staff. Per Texas Medical Board guidelines, you will be able to receive emergency care for 30 days from the date that you are notified that your membership is terminated.

HOW DO I SIGN UP?


Schedule a free meet and greet with Dr. Edwards. During this visit, you will be provided with the details of the concierge program, review of the concierge contract and the ability to ask Dr. Edwards any questions you may have about the program. Once you have been signed up, your first appointment will be scheduled.
